To help our team investigate and resolve issues quickly and effectively, please try to include the following details in your report (The 5W1H Method):

Where did the incident or offense take place?

Who was involved, and are there any witnesses who can assist?

What exactly happened, or what offense was committed?

Why did the incident or offense happen?

How was the incident or offense carried out?
